8. the address of your main website if you have one and if you want to link to it - [optional]
9. addresses of your blog, music and/or photo websites
10. a contact name (or two) - [optional]
11. fixed telephone number - [optional]
12. fax number - [optional]
13. mobile telephone number - [optional]

*An asterisk at the end of a line means you MUST supply this information.

You can also send (separately if using this email form which does not take attachments) the following extras.

1. a photograph/image which we will make into the small logo/graphic at the top of the page
